Cobber Sports Medicine

Concordia has one of the most comprehensive sports medicine department in the area. Concordia has two full-time athletic trainers on staff and has full access to the Sanford Orthopedic Department.

All current Concordia student/athletes are required to fill out medical, insurance and participation forms. Click on the links below to access all the necessary information and forms for the Cobber Athletic Training staff.

Freshman and Transfer Students
All freshman and transfer student/athletes must read and complete these forms:

- Letter Regarding Insurance Information

- Medical Coverage Information

- First-Year Medical Participation Form

- Insurance Information Form (must be completed annually)

- Acknowledgement of Insurance Form (must be completed annually)

- Banned Stimulant Medication Reporting & Guidelines For ADHD Treatment

Returning Students
All returning student/athletes must read and complete these forms:
